to questionnaires without disclosing any personally
identifying information. The web site cannot link your
answers to any kind of personal identification.
While you are using Disease Risk Index, the
information you supply about yourself is temporarily
stored in a secured text file. As soon as you exit the
site, that information is permanently deleted. If you
choose to give us feedback about the site (for example,
telling us whether you think the explanations given
are clear), we will keep that feedback and use it to
improve the site.
Disease Risk Index asks you to provide information
about your health only for the purpose of generating a
personally tailored description of your disease risk. It
does not sell, rent or lease any of the information you
supply.
